Understanding the Difference between Prophy and Scaling and Root Planing

Introduction:

During the initial exam, Dr. Toan Ho and the hygienist at Thrive Dental Care work together to evaluate the oral health of their patients. The hygienist will begin by evaluating the periodontal health or the gums. After a diagnosis is made by Dr. Ho, one of two procedures are recommended for the patient: 1) dental prophylaxis (cleaning or prophy) or 2) scaling and root planing (deep cleaning). While both aim to improve oral health, they serve different purposes and are used under specific circumstances. In this article, we will explore the key differences between prophy and scaling and root planing.

Diagnosis:

In order to determine which procedure is appropriate for any patient, a diagnosis is required. Medical history and information are gathered by the dental assistant and hygienist.  Measurements are taken by probing between the teeth and gums to determine the pockets depth.  The amount of bleeding, plaque, tartar, inflammation of the gums are also evaluated. Dr. Ho will take this information, along with evaluating the radiographs, to make a diagnosis. If the patient is diagnosed with gingivitis, a prophy is recommended. However, if the patient is diagnosed with periodontitis or gum disease, a deep cleaning is recommended. 

Dental Prophylaxis (prophy or regular cleaning):

Dental prophylaxis is a routine dental cleaning procedure performed by dental hygienists. Prophy is a preventive measure aimed for those with gingivitis or slight inflammation of the gums. The goal of this procedure is to maintain oral health and prevent dental issues like cavities and gum disease. Plaque and tartar are removed from the teeth and gumline. Polishing is then completed to remove surface stains, leaving the teeth feeling smooth and clean. Last, flossing is done to clean between the teeth, where cavities often start.

Scaling and Root Planing (deep cleaning):

Scaling and root planing, also known as deep cleaning, is a more intensive dental procedure typically performed when a patient shows signs of gum disease (periodontitis). Gum disease occurs when plaque and tartar build up below the gumline, causing inflammation, infection, and ultimately causing bone loss around the teeth. As more bone loss occurs, the tooth can become loose or mobile. The goal of a deep cleaning is to prevent further bone loss from occuring. During this procedure, the hygienist or doctor will often administer local anesthesia to ensure patient comfort. The hygienist will then begin scaling, which involves removing plaque and tartar from the surfaces of the teeth, both above and below the gumline. Root planing is then completed to smooth any rough or infected surfaces. Multiple appointments are often needed to complete the procedure.

Conclusion:

In summary, prophy and scaling and root planing are both essential procedures in maintaining good oral health, but they serve different purposes. Prophy is a routine cleaning designed to prevent dental issues and maintain oral hygiene, while scaling and root planing is a more involved procedure used to treat gum disease. Regular dental checkups, along with proper oral hygiene practices at home, can help prevent the need for extensive treatments like SRP and contribute to a healthy and confident smile.
